Passport Acceptance Services
The City Clerk Department of the City of Redwood City is
a
designated United States Department of State Passport Acceptance
Office. Recorded information is available by calling (650)
780-7588.
Location:
The City Clerk Passport Acceptance Facility is located on
the 2nd floor of City Hall at 1017 Middlefield Road, Redwood
City, CA 94063. Metered parking is available behind City
Hall. Turn right into the driveway between City Hall and
the Post Office for entrance into the parking lot.
Hours:
Effective Monday, February 4, 2013:
Extended bilingual hours for City Clerk Department Passport counter hours:
Monday from 1 to 4 p.m., Tuesday and Wednesday (added a.m. hours) from 9 a.m. to 4 p.m..
Thursdays, Fridays and holidays excluded.
Fees for new U.S. Passports Books:
Passport book application fees and the security surcharge are
payable by check or money order made payable to the Department
of State. The fee schedule is as follows:

Fees for new U.S. Passport Cards:
Effective February 1, 2008, customers now have the option to apply for a Passport Book, a Passport Card, or Both on the DS-11, DS-82 and DS-5504 forms. The Passport Card may only be used for land and sea travel between the United States, Canada, Mexico, Bermuda, and the Caribbean. The Passport Card provides a less expensive, smaller, and more convenient alternative to those who travel to these destinations.
Passport Card application fees and the security surcharge are payable by check or money order payable to the Department of State. The fees schedule is as follows:

A second required fee is the passport execution and mailing
fee. Passport execution and mailing fees are payable by cash,
check, or money order made payable to the City of Redwood
City.

Passport renewals:
Renewal forms are available at our office for applicants’ convenience;
the applicant mails the renewal form directly to the National
Passport Center. Renewal fees are payable by check or money
order made payable to the Department of State. The form may
also be downloaded from www.travel.state.gov

Passports for Children under the Age of 16:
Effective February 1, 2004, all applicants, including children
must be present when applying for a U.S. Passport.
Both parents must be present and show identification when
applying for their child's passport if the child is under
the age of 16. Effective November 1, 2004, if one parent
cannot be present when applying for their child¹s passport,
their consent must be notarized on either the "Statement
of Consent" form (DS-3053), or in a separate letter.
Passport Acceptance Service agents will not be able to process
U.S. Passport Applications unless this document is notarized.

for more specifics on applying for passports for children
under the age of 16.
Separate passport applications and fees are required for
each child for which a passport is being applied.

Note - expedited service costs an additional $60.00
per application payable by check or money order to the Department
of State. Two-way
overnight mail is available for an additional fee.
